Member Disclosure of a Criminal Conviction

A member who wishes to disclose a criminal conviction to the Institute may do so in confidence, to the Head Office, using one of the following methods:

2. Contact the Executive Director by phone at 613-236-8196 ext. 108 or by e-mail at [email protected]. Please note that a disclosure in writing will still be required. 

All information provided to the Institute will be kept confidential (including names) throughout the review and assessment process conducted by the Professionalism and Credential Monitoring Board. However, note that, by nature, a criminal conviction is public information and therefore names could become known through other sources. 
 
An official acknowledgement of receipt of the disclosure will be provided to the member within five business days of its receipt. The review and assessment process will normally be completed within 30 days of receipt of the disclosure. 
  
Questions regarding the disclosure, review and assessment process can be directed in confidence to the Professional Practice Department or to the Executive Director at the coordinates noted above.
